Saying goodbye is the hardest part of loving a pet. At Locke A. Taylor Veterinary Associates, we understand how deep the bond is between you and your companion. That’s why we’re here, to walk with you through this difficult time with empathy, guidance, and support.

Thinking about your pet’s final days is never easy, but preparing ahead of time can make all the difference. We encourage you to speak with our veterinary team before it becomes an urgent or overwhelming decision. Our team is always here to discuss your concerns, answer your questions, and help you determine when it may be time to say goodbye.

To help ease the emotional burden, we recommend discussing and documenting your preferences in advance. This includes:

  • Speaking with our team about your pet’s quality of life
  • Learning about your options for cremation or burial
  • Making important decisions before a crisis arises
  • Asking us to keep your wishes on file for when the time comes

Quality of Life Assessment: The H5M2 Scale

The H5M2 Quality of Life Scale offers a simple, thoughtful way to evaluate your pet’s well-being.

Each of the seven categories is rated from 0 (poor) to 10 (excellent). A score of 35 or higher generally indicates a reasonable quality of life, but each pet and situation is unique, our team is here to help interpret these results with you.

Hurt

Is your pet in pain? Can discomfort be managed with medication or other treatments?

Hunger

Is your pet eating regularly and willingly? Are they able to maintain a healthy weight?

Hydration

Is your pet drinking enough water? Are they dehydrated or relying on subcutaneous fluids?

Hygiene

Can your pet groom themselves or be kept clean without distress? Are they incontinent?

Happiness

Does your pet still show interest in their surroundings, toys, family, or walks?

Mobility

Can your pet move around without significant pain or distress? Can they stand, walk, or use the bathroom unassisted?

More Good Days than Bad

Does your pet still have more good days than bad ones? Are the bad days increasing?
